First, follow these additional instructions on optimizing life span and battery life prior to submitting your iPhone for battery replacement.
Note: Apple may not be able to service the device if it's damaged due to abuse or accident. You can review Apple's Repair Conditions and Terms for more information.

You can take your iPhone into your local Apple Retail Store or contact Apple Technical Support and ask about the iPhone Battery Replacement Program.
How much does it cost?
The iPhone Battery Replacement Program costs $79.00, plus an additional $6.95 for shipping. Total cost per iPhone is $85.95 and all fees listed are US and subject to local taxes.
Is the data on my iPhone saved?
No, the iPhone battery replacement service will delete all data from your iPhone. It's critical that you sync the iPhone with iTunes and backup all email settings, contacts, and photos prior to mailing in the device. Apple is not responsible for the loss of information while servicing the iPhone and data transfer services are not available. Also, do not mail any accessories with your iPhone as these will not be returned.
How long does it take to complete the service?
